For some weird reason, it was disabled in Extension Manager in Visual Studio 2010 (but not in Visual Studio 2012) when I installed it. I re-enabled it. The first time it took a long time to load VS2010 (10-15 seconds vs. 2-3 seconds). The second time I loaded VS2010 it seems faster now. Strange.

FYI...I installed it from the stand-alone EXE (i.e. outside VS2010 and VS2012.

I installed the EXE on two (2) separate machines -- one running Windows 7 Ultimate 32-bit and the other running Windows 7 Ultimate 64-bit. Both had VS2010 and VS2012 installed. On both the machines, the SQL Server Compact Toolbox was disabled once it was installed to VS2010 but enabled on VS2012. I fixed this via Extension Manager. Just thought you should know.
